Missed Appointments
Repeated missed appointments is a pattern we can not ignore. This behavior is an immense inconvenience to our patients as well as our staff. You may be subject to a missed appointment fee of $50.00.
Motor Vehicle Accident
If you are scheduled for an injury which is the result of a motor vehicle accident please bring all the required insurance information, including the name of the carrier and claim number.
Scheduling
Our office hours are by appointment only. When calling for an appointment please furnish our staff with the reason for your impending visit. This will allow us to give priority to your needs and reserve sufficient time for your visit. When acutely ill, call the office as early as possible.
If you are unable to make your scheduled appointment, please let us know at least 24 hours in advance.
Check In / Registration
Please use our new check-in link sent directly to your phone as a text message. Once checked in, our staff is notified that you have arrived at the office. Please stop at the front window to pay your copay and then you can have a seat in the waiting room.
Please bring a list of your current medications. We will provide you with a form to confirm your current medications and add new ones which may have been prescribed to you by an outside specialist.
Check-Out
Please stop at the front window on your way out to schedule your appointment for next year, otherwise a member of our staff will contact you to schedule your annual appointment.
Workers' Compensation
Effective October 1, 2014, Quaker Medical Associates will no longer be treating injuries or illness related to workers' compensation.

A Patient-Center Medical Home is a team based model of care led by a personal physician who provides continuous and coordinated care throughout a patient's lifetime to maximize health outcomes. Immunization
Vaccination is the single most important opportunity to prevent serious illness in your child, including brain damage and death. Leaving your child unprotected against vaccine-preventable illnesses poses a great risk to his or her health. If you have concerns about childhood vaccines, we would be happy to discuss the risks and benefits of vaccinating versus not vaccinating your child.
If you have chosen not to vaccinate your child, our pediatric practice may not be the right fit for your family. We feel strongly that vaccinating your child is one of the most important things that can be done to safeguard your child's health. Since we will be partners in making sure your children remain healthy, it is important that we have similar philosophies in this regard.
Behavioral Health
Our office provides behavioral health services, using evidence-based practices in screening for and treating depression, anxiety, and other conditions that can be effectively managed by our providers. If a patient does not respond to our treatment, our providers will direct the patient to the appropriate behavioral health specialist.

School Health Forms
If your child will need medical forms for camp or school, make sure they will have had a well visit within the past year. Well visit appointments fill up months in advance, so make sure you call well in advance to schedule. If your child has had a well visit in the past 12 months, we can complete a form based on the last well visit.
Please give our office 7-10 business days to complete these forms. If you require expedited service we may be able to accommodate your request, if your physician is in the office that day. There is a $50 service fee for same day form completion.

Incomplete forms may cause a delay in receipt of results. Completed forms can be emailed to nbsinfo@health.ny.gov or faxed to (518) 474-0405. The newborn screening program must be in possession of a signed form in order to process requests. Verbal requests are not accepted.
Please note that these requests may take up to two weeks for NYS DOH to retrieve the record from their archives.
